Existing information systems offer only partial or inadequate solutions. Numerous web-based systems allow patients to store and request personal medical information. However, because such information is provided by patients, its content cannot be trusted by physicians as of liability reasons. A reliable system must contain data input by medical professionals to be verified by their digital signatures.

Some island solution like systems, such as "smart card networks", contain information entered by qualified personnel working within a specific medical organization, but lack interoperability with outside providers.
